Bladder tumour surgery Overview:
Bladder tumour surgery is part of the treatment for bladder cancer. The surgery type depends on the extent of cancer. It also depends on the choices you made that will leave on the surgery.
Transurethral resection of bladder tumor (TURBT)
- A TURBT or a transurethral resection TUR helps in finding if someone has bladder cancer, and if so, whether cancer has spread to the bladder wall muscle layer.
TURBT is the common treatment for patients at the early-stage of bladder cancer. In most cases, this is the first treatment. In some cases, TURBT is done to ensure that cancer is removed. The treatment helps to remove cancer cells along with nearby tissues.
How TURBT is performed?
- The surgery is done with an instrument, and it is put in the urethra. The patient is either given regional anesthesia or general anesthesia. A rigid and thin cystoscope is put into the bladder through the urethra and it is known as a resectoscope. The instrument has a wire loop which helps in taking away the tumor or abnormal tissues. Once the tissues are taken, they are sent to the lab for testing.
Once a tumor is removed the doctor will perform different steps to ensure cancer is not present. The fulguration is performed in which tissue is taken, and the tumor is burned. It might be possible cancer cells are destroyed with high-energy lasers.
What are the side effects?
The TURBT side effects are mild and go away with time. After the procedure, there might be bleeding and pain during urination. You can get back home on the same day or the next day. To get back to the normal activity you need one week or two. In some cases, bladder cancer can come back to other bladder parts. In that situation, another TURBT is needed.
Patients with non-invasive low-grade and recurrent history of tumors, there is a need for fulguration to burn small tumors. The patient is given local anesthesia, and it can be slightly uncomfortable.
Cystectomy
If the situation is worse, then all the bladder parts need to be removed, and this is performed with cystectomy. Before cystectomy, chemotherapy needs to be performed. The patient is given general anesthesia to reduce discomfort.
Partial cystectomy
If cancer has reached the bladder wall but not large, then the part of the bladder wall is removed without taking the entire bladder. Stitches are done to close the bladder wall.
The primary benefit is there is no need for reconstructive surgery, and the patient will have their bladder. The rest of the bladder will not hold the same amount of urine, and they need to urinate often.
Radical cystectomy
If the cancer is large and has different bladder parts then radical cystectomy is true. The surgery helps in removing nearby lymph nodes and the entire bladder.
In the case of men, seminal vesicles and prostates are removed. In the case of women. The uterus, cervix, fallopian tubes, and a small part of the vagina are removed.
